Charles O. “Chuck” Lee

President & Chief Executive Officer

Before joining Cornerstone Hospice as President and Chief Executive Officer in 2013, Chuck had more than 11 years of executive-level hospice experience in developing and executing strategic plans, building leaders, expanding into new markets, increasing patient referrals and daily census, and enhancing volunteer and fund-raising efforts.Chuck holds a Bachelor of Arts, magna cum laude, from Loyola University in New Orleans, LA. He earned his Master of Arts at The George Washington University, Washington, DC, and his Master of Business Administration at the University of Wisconsin-Madison, Madison, WI. Also, Chuck has completed an extended internship in Clinical Pastoral Education at Baptist Hospital in Pensacola FL, and he is a Permanent Deacon, ordained in the Roman Catholic Diocese of Pensacola-Tallahassee.

In 2009, Nick came to Cornerstone Hospice after 25 years of fundraising experience at the American Heart Association and, more recently, with the University of Miami, School of Medicine. Nick is responsible for providing leadership for support of the valuable services and programs offered by Cornerstone Hospice. As donations to the Foundation are translated into better care for all people in our area affected by serious illness, improving people's lives becomes a realization.

Jeanne Mulkeen
Vice President, Human Resources
Jeanne joined Cornerstone Hospice in August, 2013. Her experience includes more than 20 years of HR leadership with 10 years in the healthcare industry where she led teams in human resources, employee health, volunteer services and physician practices. She most recently served as the VP of Human Resources at Livingston Services Corporation in New Jersey. As a Senior Professional in Human Resources, her role at Cornerstone Hospice will direct all of the people functions for the interdisciplinary teams of medical practitioners, social workers, chaplains and support staff dedicated to serving patients in the seven-county territory. Additionally, she is responsible for translating the organization’s strategic plans into operational ones.

Vice President, Executive Director - Lake/Sumter Counties
Deb Harley joined Cornerstone Hospice in early 2010 with a great deal of Hospice experience. Previously, she was the CEO for Hospice of Volusia Flagler for many years.

Executive Director - Polk, Hardee, Highlands, Orange, Osceola Counties
Before joining Cornerstone Hospice in July 2011, Violet had 18 years of hospice experience.

Rhonda White
Vice President, Chief Operations Officer
Rhonda had 10 years of experience in hospice and palliative care when she came to Cornerstone Hospice in 2008. She first began her career as a Social Worker then moved to the field of Compliance in 2001. As the VP of Quality & Compliance, she oversaw the organization's efforts to comply with the Laws and Regulations. She monitored and evaluated our performance improvement efforts and helped the organization find new and creative ways to constantly improve quality. Prior to being named COO, she supervised the Health Information Management, Quality, and Risk Management Departments also. As COO Rhonda provides overall management and guidance for patient and family services for the agency.

Cindi Harris PanningUrsula Cutler, RN
Vice President, Sales and Marketing
Upon arriving at Cornerstone Hospice in 2014, Ursula had had a long and varied career in healthcare and healthcare marketing. She has worked at hospice agencies in clinical and marketing roles. At Cornerstone, she oversees staff dedicated to clinical and community outreach. Additionally she manages all advertising and marketing outreach as well as educational collateral creative.

Donna Loyko, RN, MSN, HCRM, CHC
VP, Compliance Officer
After serving as Risk Manager, Donna was named Compliance Officer in early 2014 and has more than 30 years nursing experience, Risk Management and leadership in home care, hospice and corrections health care. She monitors Cornerstone departments to ensure compliance and quality measures. She helps identify and implement best practices across the organzation. She has a Bachelor's degree in Nursing from Florida Southern College and a Master's degree from the University of Phoenix. She received her Risk Management license from the University of Central Florida.

Jack Schiavi RN, CHPN
Executive Manager - Hospice Houses and Inpatient Units
Jack has been a nurse over 30 years with prior Oncology and Critical Care back round. Beginning as a volunteer with Hospice of Dutchess County NY in 1987, Jack moved to Florida in 1991 to pursue his career in hospice. Jack joined Cornerstone Hospice in 1998 after working at two other hospices in Orlando. Jack graduated from Colombia Presbyterian University School of Nursing and is currently a Certified Hospice and Palliative Care nurse. Jack has worked in many areas of Cornerstone Hospice including managing the Mike Conley Hospice House Inpatient Unit located in Clermont. Jack was named Executive Manager for all Cornerstone Hospice house and inpatient units in November, 2013.

Julia A. Allen
Community Relations Manager
Julia joined the Operations Board of Directors in 2001 and served as its Executive Secretary in 2003. Formerly Physician Relations and Services Director for The Villages Regional Medical Center, in January 2004 she joined the Cornerstone as a Community Liaison and in February 2005 was named Community Relations Manager. She is a 2006 Leadership Lake graduate and 2013 Leadership Sumter graduate. She is responsible for designated public and community relations outreach activities as well as community education, audio/video production, media and market research in all seven counties served by Cornerstone Hospice.

Lori Allen, RN
Manager - Lane Purcell Hospice House
Lori has been with Cornerstone Hospice since June 2003. Her prior experience is team nurse on the Red/Villages team. She was named LPHH manager in the fall of 2013.

Richard Behers, DMin.
Spiritual Care Manager
Rich came to Cornerstone Hospice in January of 2012. He is credentialed as a Board Certified Chaplain by the Association of Professional Chaplains and a Board Certified Clinical Chaplain and Fellow in Hospice and Palliative Care by the College of Pastoral Supervision and Psychotherapy. Rich served 25 years as a Senior Pastor prior to entering hospice care in 2006.

F. Marcella Bland, RN
Tavares Hospice House Manager
Marcella coordinates and manages the Tavares/Descipio Hospice House. She began her association with Hospice of Lake & Sumter/Cornerstone Hospice and Palliative Care in 1986 as a community volunteer serving on the Quality Assurance Committee. Marcella joined the organization in 1991 as Coordinator Special Projects working in many aspects of the growth and development of hospice services to Lake and Sumter communities. One special project, that will be forever a credit to the organization, was building and opening (1991) the first free-standing hospice inpatient unit to serve terminally-ill patients and their families. In addition to the years of managing hospice house, Marcella’s nursing management experiences include acute care, sub acute and rehabilitation care.

BJ Coulliette, RN, MHA
QA & Compliance Director
BJ came to Cornerstone Hospice in 2012, after working for Central Florida Health Alliance for 24 years. She has extensive experience in Risk Management, Infection Control, Education, Patient and General Safety. BJ is a Licensed Health Care Risk Manager in the State of Florida and also holds her national certification as a CPHRM.

Carolyn has worked at Mike Conley since the opening in 2007.

Lisa Gray
Volunteer Services Manager
Lisa joined Cornerstone Hospice in 1993 as a Team Assistant, a position she held for seven years. As the organization grew, so did the Volunteer Services area and the need for multiple Volunteer Specialists. In 2001 Lisa became a Volunteer Specialist. In that position she headed the Teens for Hospice working closely with middle and high school student volunteers. She now manages the Volunteer Specialists in all seven counties served by Cornerstone Hospice.

D. J. Hamilton, CSM
Information Technology Director
DJ came to Cornerstone Hospice with fifteen years of experience in the computer technology field in 2000. The Information Technology department is charged with the responsibility of supporting and providing the organization with solutions to effectively and efficiently meet its current and future computer and related needs.

Ann Harr
, R.N.
Regional Team Manager - Orange & Osceola Counties
Ann has been an RN for over 20 years with experience in Internal Medicine, Dialysis, Blood Banking and hospice. Prior to joining Cornerstone in 2005, she was with VITAS for 9 years. During her hospice tenure, she has held a variety of positions including Admissions Nurse, Admissions Manager, Primary RN, Team Manager, Crisis Care Manager and, most recently, Patient Care Supervisor for Cornerstone Hospice.

Cindy Hoover
Executive Assistant
Cindy joined the Hospice staff in 2000 as Executive Assistant to the CEO. She has more than 16 years of experience in hospice development and administration. She is past Membership Chair of the Grant Professionals Network and a past Ambassador of the Winter Park Chamber of Commerce.

Patricia Ludemann, RN
Regional Team Manager, Polk, Hardee and Highlands Counties
Patricia came to Cornerstone Hospice in November 2004. For sixteen years prior she had been the Manager for Home Health United in Wisconsin. As the Regional Team Manager in Polk, Highlands, and Hardee Counties, she is responsible for team staffing and education, patient care, coordination with community activities, and ensures her team is fiscally responsible.
Dan McCormick
Director of Support Services
Dan joined Cornerstone Hospice in March 2005. With over 30 years experience in management in addition to overseeing commercial, residential construction and facilities management prior to joining Cornerstone, Dan has put his experience in the non-profit community in management, community relations and fund development.
Jill McKinnie, RN, BS, CHPN
HIM Manager
Jill joined Cornerstone Hospice in 1987 and has served as a Nurse, Regional Team Manager and Education Coordinator. She now uses her clinical skills combined with her expertise in information technology to train field staff in clinically documenting patient activity via laptop computers.
Michael E. Metcalf, BA, PHR
Human Resources Managerr
Mike has 13 years experience in the Human Resources field. Prior to joining Cornerstone Hospice he was the Human Resources Director with Presbyterian Village North, a Continuous Care Retirement Center in Dallas, Texas. Mike is the Diversity Chair with the Lake-Sumter Society for Human Resources Management as well as serving as a member of the Cornerstone Hospice Diversity Council. Mike has fiduciary responsibility for the Cornerstone Hospice 403(b) Employee Retirement Plan. Among his other responsibilities are oversight of compensation, benefits, employee relations, recruitment and workforce planning and employment.

Carlos Palomino, RN
Admissions Manager - Orange/Osceola
Carlos came to Cornerstone Hospice in 2011. His background is in Psych Med, Medical Telemetry and Pediatrics. He has worked in the hospital setting for several years and has done home health care with pediatrics. He graduated from Florida Hospital College of Health Sciences and is working on a second degree in nursing at the University of Central Florida. As admissions manager, he supervises RNs who cover 15+ hospitals in the vicinity. His role also includes maintaining good partnership with physician practices, Assisted Living Facilities and Skilled Nursing Facilities.
Loretta Pruett, RN CHPN
Staffing Pool Manager
Loretta’s career with Cornerstone began in 1991 as a case manager. Prior to coming to hospice she worked in home health, assisted living facilities, hospital and office settings. She made the transition from field staff to supervisor in 2004 when she took the position of Supervisor of the Pool. She is now Manager of the Cornerstone Hospice Staffing Pool.

Julie Sneed, BSN, RN, CHPN
Education Manager - Internal
Julie started with Cornerstone Hospice in 1992 and has 20 years of Hospice experience as a Case Manager, Clinical Liaison, Clinician, Hospice House Staff Nurse, and 6 years as a Clinical Educator prior to being named Education Manager in 2012. She has a bachelor's degree from The University of Central Florida. Julie is Nationally Board Certified In Hospice & Palliative Care and is a Hospice & Palliative Nurses Association Approved Educator, ELNEC Certified Trainer, Member of Lake Sumter State College Nursing Advisory Board, and Member of Lake Technical Nursing Advisory Board. Her prior nursing experience includes Oncology Nursing and Pediatrics. The Education Department is responsible for staff development and all internal clinical education.

Emilie Van Dusen, RN, CHPN
Regional Team Manager - Blue/Aqua Teams, Lake County
Emilie began her hospice career in 1988 and is a certified hospice nurse. She came to Cornerstone Hospice in 2001 and has been the Medical Records/QI/Compliance Manager and On-call Manager. She is now the Regional Team Manager for homecare teams located in central Lake County.
Fran Wark, RN
Manager of Villages Hospice House
Fran came to Cornerstone Hospice in November 2013 with experience as a hospice Inpatient Unit Manager. She has a long work history in other clinical settings.
Charlotte Jordan, RN
Yellow Team Manager/Lake & Sumter Counties
In June 2003, Charlotte joined Cornerstone Hospice and helped open the VHH in July 2003. Prior experience included 10 years for an OB/GYN in Lake County as well as having worked in the Emergency Room for LRMC. At Cornerstone Hospice, she has worked in many nursing capacities, including admissions, clinicians and on-call. and Admissions Manager.
