Steps to Becoming a Hospice Patient
  

Becoming a Hospice patient is simple.

 

The first step takes place when a patient is diagnosed with a serious, complex or advanced chronic illness or injury. The patient’s doctor can refer him or her to Cornerstone Hospice and Palliative Care by signing a form stating that the patient has months, rather than years, to live, and by identifying the patient’s diagnosis.

Next, a Cornerstone Hospice admissions’ nurse evaluates the patient and consults with his or her physician. The nurse gets the process moving – ordering supplies and medications according to the physician’s orders.

From there, Hospice’s interdisciplinary team provides caring, end-of-life services to the patient and family. 

 

Getting admitted. . .

If a patient feels he or she is ready for Hospice, but his physician has not suggested it, a patient or those who care about him or her, may contact Cornerstone Hospice at 352-742-6809 or Toll Free at 866-742-6655 or email admissions@cornerstonehospice.org.

 

Cornerstone Hospice will contact the physician asking for the certification form to be signed. If the patient doesn’t have a physician, our admissions department can help obtain one.  Once the certification is signed, the admissions nurse starts the admission process.

 

Making plans . . .

A plan of care is created, taking into account the patient’s medical, emotional, physical, social and spiritual needs.  The individualized plan of care is constantly adjusted as patient’s needs fluctuate.

 

The patient stays in control of his/her care . . .

A patient may withdraw from Hospice care at any time after starting the program and is welcomed back when symptoms are appropriate.
