Who is Eligible?
  

Hospice care is available to anyone approaching the end of life - at any age - home bound or not - with any life-limiting illness.

To be eligible for Cornerstone Hospice and Palliative Care services, a patient   

  • Is required to live, permanently or temporarily, in our coverage area: Lake, Sumter, Polk, Hardee, Highlands, Orange or Osceola counties. Legal residency is not required.
  • Must have an estimated life expectancy of one year or less, certified by a physician, and  
  • Must have a physician's order to receive our services.

 

Our not-for-profit organization has helped thousands of patients and their families. Since 1984, Cornerstone Hospice has made its services available to all terminally ill persons and their families without regard to age, gender, national origin, sexual orientation, disability, diagnosis, cost of therapy, ability to pay, or life circumstances. We do not impose any value or belief system on our patients or their families and respect the values and belief systems of our patients and their families.
