Our care is usually provided within the familiar setting of home -- wherever the patient calls home in our coverage area.  However, the patient does not have to be home-bound. Our care is provided in the following locations:

•   Patient's home
•   Nursing homes
•   Assisted living facilities
•   Hospitals
•   Independent living facilities
•   Inpatient units / beds in contracted skilled nursing homes
•   Hospice houses
  
•   Wherever the Patient Lives in our coverage area
