When a patient is admitted to the Cornerstone Hospice program, a Hospice admissions nurse visits the patient to get the process started according to the individual’s plan of care.  Needed supplies, medical equipment and medications are ordered and the interdisciplinary team is assigned. The family is taught how to care for the patient when Hospice is not there.  A very detailed handbook of instructions, guidelines and inspirations is left with the family to remind the family of what to do, who and when to call, among other pertinent information.  Click Patient Family Handbook to view it.
