| Creative & Caring Connection: This fund enhances the quality of care for those at the end of life. Funds can help support patients’ last wishes; expressive art, music, or pet therapy; life review; and countless other ways to help patients and their families. Regional Support: This fund helps support hospice care at home, in facilities, or at one of our Hospice Houses, in each of our service areas. These areas include Tavares, The Villages, Clermont, Sumterville, Orange County, Osceola County, Polk County, Hardee County, and Highland County. Children’s Program & Community Bereavement: When the innocence of a child’s life is invaded by the harsh realities of advanced illness—their own or that of a family member—special help is needed. Bereavement services are available to adults and children experiencing grief, even if no relative or friend was on our Cornerstone Hospice program. Supporting groups, private counseling, and even an overnight camp experience for children and teens make Hospice the leading resource in addressing grief and loss. Education Fund: Cornerstone is committed to providing the highest level of care for patients. This fund provides support for educational opportunities for staff and volunteers. Pathways: This fund ensures support for Cornerstone’s transitional program that provides limited case management for an evaluative period until a patient is ready or able to receive full hospice services or no longer needs hospice care. This program is supported 100% by the Cornerstone Hospice Foundation. Humanitarian Fund: This fund is the safety net that helps families without the financial means to meet the cost of necessities of daily life, providing emergency relief to those referred by our team of social workers. Expenses may include utility payments, food, cremation/burial services, or veteran support. Donors may also simply designate their gift to the area of greatest need.
