Hospice Patients Visiting Central Florida
  

Often Hospice patients wish to visit family and friends or to fulfill the dream of a trip to Central Florida to see one of the many attractions – Disney, Universal, Sea World to name a few.  Cornerstone Hospice can assist through a simple, temporary “vacation” transfer; the patient remains an active Hospice patient with his/her current, home Hospice for a period of up to 10 or 12 days. 

In Central Florida, Cornerstone Hospice and Palliative Care covers the counties of Lake, Sumter, Orange, Osceola, Polk, Hardee and Highlands, which includes the major cities of Orlando, Kissimmee/St. Cloud, Lakeland, Winter Haven, Haines City, Sebring, The Villages/Lady Lake, Leesburg, Clermont, and Bushnell and the surrounding towns.

Once notified that a “Vacation Contract” is of interest, a nurse from our admissions department contacts the home Hospice of the patient to arrange the “Vacation Contract” and plan for the patient’s care.
